If Death Occurs At Home
- Telephone the doctor who will visit to confirm that death has taken place.
- Contact a funeral director.
- Arrange to collect a copy of the doctor's Medical Certificate of Death (usually from the surgery).
- Speak to the funeral director or contact the Registrars Office on 03451 550077 to arrange an appointment to register the death.
- The Registrar will arrange to post out a Certificate of Registration of Death (Form 14) - to be given to the funeral director so that the funeral can go ahead. They will also issue a free abbreviated death certificate. When you are registering a death, the registrar will be able to help you with advice about the certificates you might need, and about other Council services.
If The Death Occurs In Hospital
- Contact a funeral director to inform him his services are required.
- Collect the certificate from the hospital then follow 4 - 5 as above.
